Your Foreign Medical Degree Isn't Approved in Cologne, It's Approved in Münster

The Official Rule

If there is one trap worth understanding before anything else about practicing medicine in Köln with a foreign degree, it’s this: the office that actually decides your Approbation is not in Köln at all.

Before 2020, NRW’s approach to this was split across several regional authorities. The Bezirksregierung Köln’s own current page states the change plainly: “Seit dem 01.03.2020 ist für Neuanträge bei einem Abschluss der Ausbildung im Ausland (EU/Drittstaaten) nur noch die Bezirksregierung Münster zuständig,” since 1 March 2020, only the Bezirksregierung Münster is responsible for new applications where the training was completed abroad, EU or third country alike. Bezirksregierung Köln kept a role, but a narrower one: it still issues Approbationen to physicians, dentists, and pharmacists who completed their German state examination within its own Regierungsbezirk. If your degree was earned outside Germany, that exception simply doesn’t apply, no matter how long you’ve lived in Köln or how convenient the local Bezirksregierung building might seem.

The office you actually need is the Bezirksregierung Münster’s Zentrale Anerkennungsstelle für Gesundheitsberufe (ZAG-aH), at Domplatz 1-3, 48143 Münster, roughly 130 kilometers from Köln. It handles Approbation applications statewide for physicians, dentists, pharmacists, and both psychological and child/adolescent psychotherapists trained abroad, regardless of which NRW city they now live in. The fee for a decision on the Approbation itself runs 150 to 700 euros depending on how much administrative work your specific file requires, and processing time varies widely, from roughly a week for a straightforward case up to several months where more review is needed, applicants training abroad tend toward the longer end of that range.

It doesn’t stop at the application. Both of the exams that commonly sit between an application and the Approbation itself happen outside Köln too, and in two different cities from each other. The Fachsprachprüfung, the mandatory C1-level exam testing specialist medical German and communication ability rather than clinical knowledge, is administered by the Ärztekammer Nordrhein at its own building, Tersteegenstr. 9, 40474 Düsseldorf, not in Köln. The fee is 430 euros, applicants who fail can retake it, and a score of at least 60 percent (36 of 60 points) is required to pass. The Kenntnisprüfung, the separate clinical knowledge exam some third-country applicants also need, is administered by an entirely different chamber again, the Ärztekammer Westfalen-Lippe, at the Ärztehaus in Münster.

The one genuine exception to “none of this happens in Köln” is counseling, and it’s worth using before you send anything to Münster. It won’t change which office decides your Approbation, that’s fixed by the 2020 centralization regardless, but it can help you understand exactly what your specific file needs, what documentation gaps to close first, and what funding options exist, before you commit time and postage to Münster.

3. Before applying, book a free consultation with the IQ Netzwerk NRW’s Köln office (MA.i Migration und Arbeitswelt e.V., Niehler Straße 86) to review your documents and flag any gaps early.
4. Budget 150 to 700 euros for the Approbation decision fee, and plan for a processing window anywhere from about a week to several months.
5. If a Fachsprachprüfung is required, plan a trip to Düsseldorf, not Köln, for the exam itself, and budget the 430 euro fee.
